Research progress on the pharmacological effects and mechanisms of notoginsenoside R1 against myocardial ischemia-reperfusion injury

Abstract
    Myocardial ischemia/reperfusion (I/R) injury is a prevalent complication following reperfusion therapy for acute myocardial infarction. Its pathogenesis involves a complex interplay of oxidative stress, inflammatory responses, mitochondrial dysfunction, and various forms of programmed cell death. Notoginsenoside R1 (NG R1), a unique bioactive saponin derived from Panax notoginseng, has demonstrated significant cardioprotective effects against I/R injury. Mechanistically, NG R1 attenuates myocardial damage by modulating multiple pathological pathways, including the mitigation of oxidative stress and inflammation, regulation of cell death signaling, and preservation of mitochondrial function. However, the clinical translation of NG R1 as a monotherapy is hindered by its low bioavailability. NG R1 exerts synergistic protective effects when combined with other natural bioactive compounds. This review systematically summarizes the primary pharmacological effects and underlying mechanisms of NG R1 in combating myocardial I/R injury. Furthermore, we critically discuss the limitations associated with its monotherapeutic application and explore the potential of synergistic combinations. This work aims to provide a theoretical basis and reference for the development of natural drug strategies to improve outcomes in myocardial I/R injury.
