Abstract
Acute pancreatitis(AP)is characterized by acute onset and rapid progression. Severe AP is often accompanied by a systemic inflammatory response, intestinal mucosal barrier dysfunction, and distant organ injury, particularly lung injury. Therefore, identifying effective strategies for inflammatory regulation is of great significance. The NOD-like receptor family pyrin domain-containing 3 (NLRP3)inflammasome is a key regulatory component involved in the persistent amplification of inflammatory responses in AP. Active components and compound formulas of traditional Chinese medicine(TCM),which are characterized by multi-component and multi-target actions, have attracted increasing attention in recent years for their potential in modulating inflammation in AP. This review discusses the mechanisms by which TCM active components, including anthraquinones, flavonoids, phenolic acids, and other polyphenols,as well as TCM formulas such as Qingyi Decoction,Dachengqi Decoction, and Chaiqin Chengqi Decoction, regulate NLRP3 inflammasome activation. In addition, this review summarizes their effects in alleviating pancreatic injury, improving intestinal barrier function, and mitigating AP-associated lung injury. Current evidence is still mainly derived from basic experimental and preclinical studies, and clinical evidence remains relatively limited. Overall,TCM-mediated regulation of the NLRP3 inflammasome has research value and potential application prospects for AP intervention, but the relevant conclusions still require further validation in high-quality clinical studies.
