基于“湿浊痰饮类病”理论探究痰湿体质人群心力衰竭风险中的血浆脂质标志物

摘要:
[目的] 心力衰竭(HF)是心血管疾病的终末阶段,其发病率和病死率居高不下。国医大师张伯礼提出的“湿浊痰饮类病”理论对HF的病机认识及治疗具有指导意义,而HF发病风险与痰湿体质具有潜在相关性。研究旨在探讨痰湿体质人群HF风险中的血浆脂质标志物,并通过现代组学技术揭示其潜在的代谢机制。[方法] 研究基于国际公共数据库中的全基因组关联研究(GWAS)数据,应用孟德尔随机化(MR)分析方法,结合脂质组学数据,探索痰湿体质与HF之间的因果关系。通过两步MR分析,进一步计算血浆脂质标志物在痰湿体质与HF风险之间的中介效应。[结果] 研究结果表明,痰湿体质显著增加HF的发病风险(OR=1.205,P<0.001)。此外,56种血浆脂质代谢物与HF风险显著相关,其中30种脂质标志物在痰湿体质与HF风险之间起到中介作用。[结论] 研究在“湿浊痰饮类病”理论框架下,从遗传因果推断层面对痰湿体质相关代谢特征与HF风险之间的关系进行了评估,并识别了可能参与该转化过程的血浆脂质中介标志物。研究结果为中医体质相关心血管疾病风险的现代化量化研究提供了方法学参考,也为体质基础上的心血管疾病风险评估与干预策略研究提供了数据支持。

Exploration of plasma lipid biomarkers associated with heart failure risk in individuals with phlegm-dampness constitution based on the theory of “similar diseases and syndromes of damp-turbidity-phlegm-rheum”

Abstract:
[Objective] Heart failure(HF)is the terminal stage of cardiovascular diseases,with high morbidity and mortality. The theory of“similar diseases and syndromes of damp-turbidity-phlegm-rheum”proposed by renowned master of traditional Chinese medicine(TCM)ZHANG Boli,provides valuable insights into the pathogenesis and treatment of HF. According to this theory,there is a potential correlation between HF risk and phlegm-dampness constitution. This study aims to explore plasma lipid biomarkers associated with heart failure risk in individuals with phlegm-dampness constitution and reveal their potential metabolic mechanisms using modern omics technologies. [Methods] This study is based on genome-wide association study(GWAS)data from international public databases. Mendelian randomization(MR)analysis was employed,combined with lipidomics data,to investigate the causal relationship between phlegm-dampness constitution and HF. Two-step MR analysis was further conducted to calculate the mediating effects of plasma lipid biomarkers between phlegm-dampness constitution and HF risk. [Results] The results show that phlegm-dampness constitution significantly increases the risk of HF(OR=1.205,P<0.001). Furthermore,56 plasma lipid metabolites were significantly associated with HF risk,of which 30 lipid biomarkers play a mediating role between phlegm-dampness constitution and HF risk. [Conclusion] Within the framework of the“similar diseases and syndromes of damp-turbidity-phlegm-rheum”theory,this study applied genetic causal inference to evaluate the association between metabolic traits related to phlegm-dampness constitution and heart failure risk,and identified plasma lipid biomarkers with potential mediating roles. These results provide methodological support for the quantitative study of constitution-related cardiovascular risk and offer data reference for constitution-based cardiovascular risk assessment research.
