| 线粒体自噬是特异性清除受损线粒体、维持细胞稳态的关键选择性自噬过程。慢性乙型肝炎(CHB)是引发肝纤维化、肝硬化及肝癌的主要致病因素。研究表明,乙型肝炎病毒(HBV)通过劫持线粒体自噬促进病毒复制与免疫逃逸,由此引发的线粒体自噬障碍及持续性炎症会激活肝星状细胞(HSC),最终导致肝纤维化。因此,阐明线粒体自噬在CHB进展为肝纤维化过程中的双向调控机制具有重要意义。目前,中医药通过干预线粒体自噬,在抑制病毒复制、减轻炎症反应及阻断HSC活化方面已取得显著成效。文章综述了线粒体自噬在CHB向肝纤维化进展中的病理机制及中医药干预策略,旨在为CHB相关肝纤维化的精准治疗提供理论依据,并为中医药现代化研究开辟新思路。 |

| Mitophagy specifically clears damaged mitochondria to maintain cellular homeostasis. Chronic hepatitis B(CHB)is a major cause of liver fibrosis,cirrhosis,and hepatocellular carcinoma. HBV hijacks mitophagy to promote replication and immune evasion;the resulting mitophagy dysfunction and persistent inflammation activate hepatic stellate cells(HSCs),ultimately causing liver fibrosis. Thus,elucidating the bidirectional regulatory mechanisms of mitophagy in CHB progression to liver fibrosis is crucial. Currently, Traditional Chinese Medicine(TCM)shows remarkable efficacy in inhibiting viral replication,alleviating inflammation,and blocking HSC activation by targeting mitophagy. This review outlines the pathological mechanisms of mitophagy in this process and corresponding TCM strategies,aiming to provide a theoretical basis for the precision treatment of CHB-related liver fibrosis and new perspectives for TCM modernization. |
